Luis Rafael Diaz Hernandez

Luis Rafael Diaz Hernandez, age 53, passed away December 4, 2021 at Metro Hospital, Cleveland, Ohio.  Born December 14, 1967 in Juana Diaz, Puerto Rico, to the late Maximino and Maria Elena (Hernandez) Diaz, he resided in Cleveland, Ohio. 

He is survived by his children, Luis Rafael Diaz, Jr. (Ann), Anthony Diaz, Shabellie Mary Diaz and Johnny Boy Diaz; grandchildren, Kai Anthony and Aria Mildred Diaz; 8 brothers and sisters. In addition to his parents, he was preceded in death by brothers, Victor, Tom, Maximino and Pedro Diaz.  The family will receive friends at the Slone & Co. Funeral Directors, 13115 Lorain Avenue, Cleveland, Ohio, Wednesday, December 8, 2021 from 10:00 a.m. to 12:00 Noon.  Burial will follow at Riverside Cemetery, 3607 Pearl Road, Cleveland, Ohio 44109.  The family requests contributions be made in memory of Luis to the Cleveland Animal Protection League.  Clevelandapl.org
